Fees, Insurance information, and Office Procedures
Professional Fees
Fees for Dr. Greenfield's services at the Healing Center, LLC vary based upon the nature and type of service, as well as the length of the service provided. All fees are due at the time services are provided, and we do not bill for services. Dr. Greenfield does not participate in any HMO's, PPO's, of other managed health plans, however you may be able to submit you charges to you health insurance plan if you have a an "out of network benefit" of "point of service " option. You will have to speak to your Insurance company or the behavioral/mental health company that manages your medical plan. Dr. greenfield will provide you, upon your request, an itemized bill with all charges, diagnostic codes, and procedure codes suitable for your insurance company to process.
All fees for services are due and payable at the time of the visit unless other arrangements have been made prior to your appointment. Appointment times generally last 30-45 minutes for counseling and psychotherapy. Imago marriage and relationship therapy sessions and EMDR may run up to 90 minutes and fees are adjusted accordingly. Group Therpy runs 90 minutes. Some appointments are longer or shorter depending upon the type of service required. At times, telephone contacts may be required to obtain information from schools, physicians, attorneys, teachers, or other doctors/therapists. If an extended phone contact is necessary, there may be a charge for such time spent. Your doctor or therapist will inform you if this is the case. Initial consultations (first visit) or an extended telephone consultationt is typically billed at slightly higher rate. Legal consultations (in conjunction with you attorney) are billed under a separate fee schedule. Please ask Dr. Greenfield about these rates and services. Report preparation is also billed at different rates from clinical services.
We accept Master Card, Visa, American Express, and Discover for your convenience. Checks may be used. There will be a $25.00 returned check fee for all uncollected checks. We cannot bill for co-pays or deductibles all fees must be aid at the visit.
Health Insurance
Even if using out of network options your health plan may require prior authorization and approval. In addition, they may require Dr. Greenfield to complete a treatment plan or other documentation. It is your responsibility to find out what your coverage is and if you require pre-authorization or a treatment plan. We can help guide you on what to ask and whom your should contact. If you do not obtain the proper authorization then you may not receive the coverage you were hoping to use. You always have the right not to use your health insurance. Once we provide you with the necessary receipts and service bill you may submit this to you plan for reimbursement subject to the plan's limitations.
managed care
These days some type of managed care plan covers most people. If you have this type of plan, (HMO, PPO, etc.) your doctor or therapist will have to provide information to a case manager about the nature of the problems you are having along with a diagnosis and the treatment plan. to obtain authorization for treatment. Please be aware that decisions regarding approval for further sessions rest with the managed care company, though you or your doctor/therapist have the right to appeal a decision regarding your treatment if you feel a denial for further visits are unjustified. Please be advised, however, that decisions for approval of sessions are based on clinical/medical necessity rather than being based on seeking personal growth, self-improvement or optimal functioning in one’s life. If your goals fall in the latter category, and do not meet criteria for approval by your managed care plan, you can discuss this with your doctor/therapist. Most services tend to eligible for reimburse if you have an out of network option.

Missed Appointments, Cancellations and Appointment changes
Since your appointment times are reserved well in advance, you are asked to keep all scheduled appointments. We require that you provide at least 24 hours notice (preferably more) for any cancellation or change. The more notice we have, the more flexible we can be in accommodating appointment requests. If an appointment is cancelled with less than 24 hours notice, the full fee will be charged for the appointment time. Insurance companies do not reimburse you for missed/broken appointments, and you will be responsible for the full charge. We understand that bad weather conditions may require you to stay home if you feel endangered by traveling; on such occasions you and your therapist can consider the option of conducting a session by telephone If you have a circumstance or emergency that will preclude you ability to keep your appointment please contact my office immediately. If you simply want to change your appointment let us know and we will try to accommodate your needs.If you have an appointment on a monday please cancel your appointment by Friday afternoon as I do not check voice mail over the weekend.
voice mail service
Voice mail is available where you may leave a confidential message and is available 24 hours. Please listen for the message and record your reply. When you leave a message, your doctor or therapist will return your call as soon as possible. If you are having any difficulties with the voice mail system or the answering service, please let your therapist know immediately so it can be addressed. In the case of a life threatening emergency always contact 911 or call an ambulance, or proceed to the emergency room.
